Vital RX Pharmacy (“we,” “our,” or “us”) is committed to protecting your privacy. We comply with the Personal Health Information Protection Act, 2004 (PHIPA) of Ontario and the Personal Information Protection and Electronic Documents Act (PIPEDA) of Canada.

 

Your Rights

When it comes to your health information, you have certain rights under Ontario law:

  • Access your record
    You may request to see or get a copy of your health record. We will provide a copy or summary within 30 days, subject to a reasonable fee.
     

  • Request corrections
    You may ask us to correct health information that you believe is inaccurate or incomplete. If we deny your request, we will explain why in writing.
     

  • Request confidential communications
    You may ask us to contact you in a specific way (for example, only at work or through mail). We will accommodate all reasonable requests.
     

  • Request restrictions
    You may ask us not to use or share certain health information for treatment or operations. While we are not always required to agree, we will do so when legally permitted.
     

  • Get a record of disclosures
    You may ask for a list of certain disclosures of your health information we have made in the past six years, except for those related to treatment or as otherwise exempted by law.
     

  • File a complaint
    You may complain if you feel your rights have been violated. You can contact us directly, or you may contact the Information and Privacy Commissioner of Ontario (IPC) at www.ipc.on.ca. We will not retaliate against you for filing a complaint.
     

 

Your Choices

For certain types of information, you can decide how we share it:

  • Sharing information with family members or caregivers involved in your care.
     

  • Using your information for educational materials, community outreach, or health awareness campaigns.
     

  • Allowing your information to be included in any research or surveys (only with your written consent).
     

We will never use or disclose your information for marketing, fundraising, or selling purposes without your explicit consent.

Our Uses and Disclosures

We typically use or share your health information in the following ways:

  • Treatment – We use your information to fill prescriptions and coordinate your care with other licensed healthcare providers.
     

  • Operations – We use your information to manage pharmacy operations, ensure quality, and comply with regulatory obligations.
     

  • Legal Compliance – We disclose information as required by law, including to Ontario’s regulatory bodies, the Ontario College of Pharmacists, and public health authorities.
     

  • Payment and Insurance – We may share limited information with insurers or benefits providers to process claims.
     

We may also disclose information in special situations permitted by law, including:

  • Reporting adverse drug reactions or product recalls.
     

  • Preventing or addressing a serious risk to public health or safety.
     

  • Cooperating with law enforcement or court orders.
     

  • Meeting obligations under workplace safety or compensation laws.
     

  • Working with a coroner, medical examiner, or funeral director, as required.
     

 

Our Responsibilities

  • We are required by Ontario law (PHIPA) to maintain the privacy and security of your health information.
     

  • We will notify you promptly if a breach occurs that may compromise the privacy or security of your information.
     

  • We must follow the duties and practices described in this notice and provide you a copy upon request.
     

  • We will not use or share your information other than as described here unless you provide written consent. You may revoke consent at any time.
     

 

Cookies and Website Tracking

Our website currently does not use cookies or tracking tools. In the future, if cookies, analytics, or advertising pixels are used, we will update this policy and provide notice to visitors.

 

Changes to This Notice

We may change the terms of this notice at any time. Updates will apply to all information we hold. The updated version will always be available on our website and in-store.
